Making a new patient appointment

  •  To schedule a new patient appointment, you can call or stop in to provide us with the information we need to schedule you! 
  • When calling or stopping in, please have the following information ready: your personal information (name, date of birth, phone number, address), diagnosis/body part we are treating, referral information, and insurance information (insurance carrier, member ID number, group number, insurance phone number)
  • We require patients to have a referral from a Doctor within the last 30 days OR we can see you Direct Access as long as you have not been seen by a Doctor or other Health Care Professional for the same condition in the last 60 days!

initial evaluation

  • Arrive about 15 minutes early to allow for ample time to check in and fill out paperwork!
  • Bring a photo ID, insurance card(s), prescription/referral, and medication list (if applicable)
  • For an easier check-in upon arrival, fill out the new patient paperwork and bring it with you to your first appointment! There may be additional forms depending on your insurance.

follow-up appointment

  •  At the end of your initial evaluation, our therapists will recommend the frequency and duration for your follow up appointments
  • We enforce a cancellation policy and require 24 hour notice for cancellations. There is a cancellation fee of $50.00
  • Co-pays are collected at the time of service and can be paid with cash, card (we do not accept Discover or AMEX), or check
  • Follow up appointments can also be made over the phone
